The applicant must be at least 18 years old upon enrollment.
Photocopies of:
- PSA Birth Certificate
- Form 137-A / Form 138-A for high school graduates
- OTR for college graduates
- Valid ID
- Marriage Certificate (for married women)
- * ALS Certificate
All pictures must be taken with formal attire and white background.
- 2 pieces passport size picture
- 2 pieces 1x1 picture
- 1 piece 2x2 picture
- Visit the school.
- Proceed to the registrar's office.
- Inquire.
- Complete the registration form.
- Submit the necessary documents.
